Registration is only open to practices invited to sign up for the reward scheme.

Registered practices will be eligible to receive one FOC replacement identichip* for every microchip identibase receive correct and full information for.

The keeper & pets details required by identibase are;

• Microchip number
• Pets name
• Keepers first and second name
• Keepers full address
• Keepers email
• Keepers phone number

FOC stock will only be replaced when identibase receive these correct details. If we do not receive these details for a claimed microchip, we will contact you to inform you.

Details can be provided to identibase via;

• Email assisted registration
• Pet Professional Portal
• PMS - VetXML

Registration requires the practice's VetCode to be provided. If you are unsure of your VetCode please email contact@identichip.co.uk with your practice address and we will get back to you. If you do not have an identibase account please email applications@identicare.pet.

To claim the FOC microchips staff will be required to input the practices VetCode and microchip number into this form.

Stock will only be replaced when multiples of 10 of the same size and format have been reached.

Your chosen wholesaler must be provided.

Backtracking of previously implanted microchips is not permitted.

There are no cash substitutes for the FOC product.

You agree that any practice information that you provide when registering will be used by identichip for the purposes of administering the scheme.

identichip reserves the right, at any time and without prior notice, to cancel the scheme or amend these terms and conditions.

*Notification of which format and size of chip will be requested. The FOC stock will be the same as is reported as implanted.
